Donating Supplies

Please submit this form if you have medical supplies/equipment/medications to donate. Once your form is received, you will be contacted via phone or email with an appointment time to drop off at the warehouse. If you have questions about what supplies and medications we might accept, please refer to the information sheet at the top of this page.

 

PLEASE NOTE: Submitting this form does NOT automatically schedule your appointment. You will be contacted once we receive your form to CONFIRM we can take your donation and to CONFIRM an appointment time for your drop off. Please allow UP TO 3 business to receive a confirmation of an appointment time for your request. If your request is being made when our warehouse is closed, please allow additional time for a response.

 

Submitting this form indicates that you are intending to make this donation as soon as possible. If you are not intending to make this donation right away, please indicate the date you would like to schedule in the "Additional Notes" section below.

 

SUBMITTING THIS FORM DOES NOT AUTOMATICALLY SCHEDULE YOUR APPOINTMENT. YOU WILL BE CONTACTED TO CONFIRM AN APPOINTMENT TIME. Drop offs are scheduled at the warehouse AND BY APPOINTMENT ONLY.

What supplies do you have to donate? (this does not need to be an itemized list - just a general idea of the types of supplies)  

 

NOTES:

  • At this time we are not taking any dialysis supplies or oxygen tanks.

  • We are able to accept ONLY Resmed brand cpap and bipap machines manufactured after June 2021

  • Liquid and pureed nutrition must have an expiration date greater than six months from the date of donation.

We accept supplies and equipment that are currently in working order as well as clean and ready to give directly to another person in need. (ie: doesn't need new batteries, has all necessary working parts, doesn't need repair work, etc.)

We only accept medications that are in original, blister-SEALED packaging. We do not accept medications that do not have the blister seal still intact. If there is not an original, intact seal on the medication, we cannot take it. We are not a medication disposal site. Please note that most regular pharmacy bottles are NOT sealed and therefore, NOT accepted. Click HERE for an example.
